Aetna provides members with a comprehensive provider network through directly contracted and indirectly contracted providers. With more than 672,000 providers under contract in the medical and/or dental PPO products, Aetna is able to offer network access in most areas of the country (data as of 04/01/2005). Additionally, Aetna reviews individual network accessibility to see that members have appropriate access to a full complement of providers. This inclusive network strategy allows Aetna to offer members the most comprehensive network available.


AHC (Associates for Health Care) provides managed health care products and services to plan participants in Wisconsin, Illinois, Minnesota and Michigan. The Preferred Provider Organization (PPO) includes the states’ finest hospitals, physicians, and other health care specialists, with a network of 131 hospitals, 7,160 specialty physicians in 23,632 locations. AHC interfaces with 220 payors including 50 insurance companies, which serve over 7,494 corporate clients who collectively provide benefits to more than 525,435 plan participants. AHC interfaces with 300 payors including approximately 50 insurance companies, third party administrators (TPAs) and self-administered groups with 404 administrative offices. In 2001, the AHC PPO represented $720 million in business to the medical marketplace.


Alliance PPO, LLC (Alliance) is a subsidiary of Mid Atlantic Medical Services, Inc. (MAMSI). Alliance has been ranked as one of the largest PPOs in the Washington, D.C. metropolitan area by the Washington Business Journal.1 The Alliance network consists of credentialed health care professionals. One of Alliance's products, MAPSI, offers access to a behavioral Health network. Alliance and MAPSI offer you and your family prompt access to medical and behavioral health/substance abuse care. Receiving medical, behavioral health or substance abuse care from preferred physicians or health care practitioners in the Alliance network saves you money by lowering your out-of-pocket costs. The network consists of over 44,000 participating physicians and health care practitioners as well as 336 facilities throughout Maryland, Virginia, Washington, D.C., Pennsylvania, Delaware North Carolina and West Virginia.


Louisiana's largest PPO, American LIFECARE offers a comprehensive physician and hospital network. Locally owned and operated, American LIFECARE provides unparalleled access to high quality medical services in rural and metropolitan areas throughout Louisiana, Mississippi and Arkansas, as well as parts of Tennessee and Alabama.


The ArcelorMittal/USW Health Care Network ("Network"), a cooperative venture between ArcelorMittal and the United Steel Workers of America Local 1010, operates in Northwest Indiana in zip code areas beginning with 463 and 464. The Network provides you and your eligible dependents with access to an exclusive network of primary care doctors, specialists, hospitals, ambulatory/outpatient surgical facilities and other health care providers offering cost and quality advantages.


Arizona Foundation for Medical Care is a network of more than 7,000 physicians in over 11,000 locations, 73 hospitals and 480 facilities. It is one of the largest, most stable health care resources in the state of Arizona.


Intro: Arkansas Managed Care Organization (AMCO), Arkansas' largest independent PPO network. Today, Center Care is the leader in managed care for Southcentral Kentucky. The diversity of the Center Care program demonstrates the ability to offer choices people want with flexibility employers need. Commonwealth Health Corporation introduced Center Care Preferred Provider Network in 1990 in order to assist employers in managing health care costs. This network is designed for an open choice PPO plan design and offers access to both primary care and specialty physicians. Central Care has approximately 70,000 lives and over 300 clients.


Since 1993, the independence of Century's network allows employers flexibility with the administrators of their health plans without requiring employees to change providers at every juncture. DirectCare America, Inc. (DCA) is a group health Preferred Provider Organization and a member of The Chandler Group of Companies. DCA is headquartered in Stow, Ohio with additional sales and provider relations located in Columbus and Toledo, Ohio, Louisville, Kentucky and Detroit, Michigan. The Emerald Health Network, celebrating its 20th year of service, is Ohio’s largest, and most comprehensive independent Preferred Provider Organization. Founded in 1983 as a joint venture between employers, physicians and hospitals, Emerald has grown into a statewide network of over 190 hospitals, 22,000 physician locations and 1,800 ancillary sites located throughout Ohio, Northern Kentucky, Eastern Indiana and Western Pennsylvania.


Encore Health Network is one of the largest preferred provider organizations (PPO) in Indiana with additional coverage in Illinois, Kentucky, Ohio and Michigan. Encore offers more than 130 participating hospitals and over 9,000 physicians throughout its service area.


Evolutions Healthcare Systems (EHS) is one of the largest independently owned PPO networks, offering substantial discounts and operational savings to self-insured employers, insurance carriers, and third-party administrators through its direct network of local, regional, and national hospitals, physicians, and specialists. FedMed is a National Provider Network and healthcare financial services organization. Guardian offers members access to a national network of dentists, orthodontists and dental practitioners. Search the dental directory for a nearby dentist or you can check if a particular dentist is a network participant.


HCN is the largest Preferred Provider Organization in Wisconsin. The network is made up of 80 hospitals, 6,100+ physicians and other health care providers in Southeastern Wisconsin. A health care program accessed by over 4,000 employers and fourteen insurance carriers.


One of the largest managed care organizations in the central United States, HealthLink, Inc. provides managed care programs and services to employers, unions, governmental subdivisions, schools and insurance companies. North Mississippi Health Services’ Center for Business Health offers a variety of managed care plans through its Health Link programs. Focusing on employer/employee needs, Health Link provides a complete range of managed care programs to industry and business. Health Link offers a network of more than 1,400 physicians and more than 40 hospitals conveniently located throughout Mississippi and northwest Alabama.


HealthCare Preferred and SouthCare are one of the largest preferred provider organizations in the country serving over 600,000 members in sixteen states in the mid-west and southeastern United States. HealthCare Preferred operates mainly in the mid-Western and mid-Atlantic regions. SouthCare operates mainly in Southeastern United States.


Healthcare VALUE Management, Inc. (HCVM) is a network of New England hospitals, physicians, and other health care providers in Massachusetts, Connecticut, Rhode Island, Maine, Vermont and New Hampshire. Founded in 1990, HCVM is the largest independent Preferred Provider Network in the six New England states. HCVM’s mission is to be the most comprehensive network of choice available in the Region connecting members with high quality providers. Currently, our PPO Network is comprised of over 240 hospitals, 29,000 physicians and over 9,000 ancillary providers. HCVM contracts with providers on behalf of over 100 health insurance payors.


HealthReach is a provider owned PPO, founded in 1985, offered by OhioHealth Group to employers located in central, southern and northwest Ohio. As of May 1, 2001, the HealthReach PPO network includes 59 hospitals, 1,783 primary care physicians, and 4,218 specialists. These providers, who have been carefully selected, contract with OhioHealth Group to provide high-quality, cost-effective health care.


HealthSmart is currently the most comprehensive independent Texas and New Mexico PPO network available. HealthSmart also provides nationwide health care coverage through direct access and management of a nationwide network,. Currently, HealthSmart serves over a half million covered lives in Texas, New Mexico and throughout the country.


HealthSpan Inc operates two preferred provider organizations in the Greater Dayton/Springfield area and Southern/Eastern Indiana area. Healthspan puts you in touch with any of their 7000 physicians serving this regional area.


HFN Healthcare PPO Network consists of 170 hospitals, 17,463 physicians and 103 COP (certified occupational provider) worker?s compensation delivery sites located in Illinois, collar cities in Eastern Iowa and the Northwest corner of Indiana.


Idaho Physicians Network is a Preferred Provider Organization that offers an extensive statewide network of healthcare professionals who offer the highest quality care to members accessing the network through insurance companies, benefit plan administrators, and self-funded employers. IPN currently has over 1700 medical providers and 52 facilities across Idaho.


Indiana Health Network is the largest access PPO Network servicing the State of Indiana. IHN offers strong discounts and the highest level of customer service available in Indiana.

Park Nicollet’s primary service area is the Minneapolis metro and surrounding communities.


Professional Coverage Managers, Inc. (PCM) was created in 1969 by Richard C. Marx to provide an alternative domestic market for Architects' and Engineers' Professional Liability. Since that time, we have broadened the scope of our operations and now serve as underwriting managers for Architects and Engineers Professional Liability Lawyers' Professional Liability Police Professional Liability, and Child Day Care Provider Liability. ppoNEXT is a consolidation of two well known and respected companies, Preferred Health Network, and HealthStar. CHA is currently ranked third in size in the Commonwealth of Kentucky among healthcare companies, CHA Health is one of the fastest growing health care companies. QualCare, Inc. is the largest full service, provider-sponsored managed care organization owned and operated in the State of New Jersey. We are majority owned by 14 New Jersey hospital systems and physician organizations, and have more than 500,000 members currently enrolled in a full range of self-funded managed care products.


Rural Arizona Network (RAN) and Arizona Medical Network (AMN) collectively represent the most comprehensive provider network in rural and metropolitan Arizona. RAN and AMN, founded respectively in 1987 and 1983, have contracts with nearly all Arizona hospitals and more than 7,000 physicians of every specialty. RAN+AMN EPO is a provider driven network, offering quality health care services at reasonable costs. The provider leadership of the EPO believes that cost containment can be achieved without sacrificing quality and without limiting access to providers.RAN+AMN EPO serves approximately 450,000 plan members with a full range of hospital and medical services with comprehensive geographic access in all Arizona counties. Key aspects of the Rockford Health Plans is access and member choice. Choice: Rock Region Health Partners PPO incorporated two local networks and are a joint venture between Rockford Health Systems and OSF St. Anthony Medical Center. This network offers members access to a full spectrum of healthcare services at Rockford Memorial Hospital and Rockford Clinic as well as OSF St. Anthony Medical Center. Access: the network consists of providers in Northern Illinois and Southern Wisconsin.


SAGAMORE Health Network is the oldest, largest and most recognized independent PPO network in Indiana with 188 Hospitals, 700+ Ancillary Providers and 20,000+ physician locations. Our network currently serves over 800,000 Members. SAGAMORE has grown dramatically over the last few years and will continue to seek out those providers that bring value to our clients.

The San Antonio Employers' Health Alliance (SAEHA), a non-profit Texas corporation, began operations in February 1996 as a result of the belief that direct contracting between employers and providers of medical and health services is the route of health care reform. Through group purchasing power, SAEHA reduces employers' health care costs without restricting employees' choice of providers. Additionally, the employer retains complete autonomy in the design and administration of its employee health care plan.
